29 CFR 1910.95(d)(1):     When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:    a) Transfer table: On or about May 23, 2012: an employee moving pipe from on stage in the process to the next was exposed to continuous noise at a level of 106.9% (equivalent to an 8 hour TWA of 90.4) which is approximately 2.138 times the action level of 50% (equivalent to an 8 hour TWA of 85dBa) Results are based on a 466 minute sampling period conducted. Exposure calculations include a zero increment for the 14 minute period not sampled. And the employer had not developed a monitoring program to ensure employees were adequately protected from noise.    b) Cooling table: On or about May 23, 2012: an employee moving pipe from on stage in the process to the next was exposed to continuous noise at a level of 92.3% (equivalent to an 8 hour TWA of 89.4) which is approximately 1.846 times the action level of 50% (equivalent to an 8 hour TWA of 85dBa) Results are based on a 478 minute sampling period conducted. Exposure calculations include a zero increment for the 2 minute period not sampled. And the employer had not developed a monitoring program to ensure employees were adequately protected from noise.    c) Saw and Pick up Area: On or about May 23, 2012: an employee moving pipe from on stage in the process to the next was exposed to continuous noise at a level of 77.4% (equivalent to an 8 hour TWA of 88.1) which is approximately 1.548 times the action level of 50% (equivalent to an 8 hour TWA of 85dBa) Results are based on a 471 minute sampling period conducted. Exposure calculations include a zero increment for the 9 minute period not sampled. And the employer had not developed a monitoring program to ensure employees were adequately protected from noise.
