29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: JOB SITE: 11834 Flushing Drive, Maryland Heights, MO 63043, at this work site, two (2) workers were observed on top the residential roof, approximately 8 feet 5 inches eave to ground level, actively installing new roof shingles, operating pneumatic nail guns without any form of eye or face protection. The workers were exposed to a struck-by hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems: JOB SITE: 11834 Flushing Drive, Maryland Heights, MO 63043, at this work site, four (4) workers were observed on top of the residential structure, approximately 8 feet 5 inches eave to ground level, actively installing new roof shingles, wearing personal fall arrest systems, but were not tied down an anchor point with lanyard. The workers were exposed to a fall hazard. 1. ON TOP CONTRACTING, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard of 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 31596411, citation number 1, item 2, issued on 06/03/2011 and was affirmed as a final order date of 06/28/2011, with respect to a workplace located on 11630 Old St. Charles Rock Road, Bridgeton, MO 63044.
