29 CFR 1926.451(b)(2):     Scaffold platforms and walkways were not at least 18 inches wide:  (a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)      (a) Under Bridge over Small Stream on Millersburg Side - Employees worked taking down jacks.  To get from the bank to the scaffolding under the bridge, the employees walked on a single 2x10 plank.  In other words, the walkway from the bank to the scaffold had a width of 10 inches rather than the required 18 inches, on or about May 21, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
