(b) Milling Area: On or about May 23, 2012, and at times prior thereto, employees were exposed to flying chips and pieces while operating the Bridgeport milling machine, serial number J298156R, without a guard.     (c) Milling Area: On or about May 23, 2012, and at times prior thereto, employees were exposed to flying chips and pieces while operating the Sharp milling machine, serial number 60900988, without a guard.     (d) Milling Area: On or about May 23, 2012, and at times prior thereto, employees were exposed to flying chips and pieces while operating the Sharp milling machine, serial number 70502978, without a guard.    (e) Milling Area: On or about May 23, 2012, and at times prior thereto, employees were exposed to the hazard of becoming caught in the rotating chuck while operating the Sharp lathe, serial number 2939, without a guard.     (f) Milling Area: On or about May 23, 2012, and at times prior thereto, employees were exposed to the hazard of becoming caught in the rotating chuck while operating the Nardini lathe, ND 1560, without a guard.
