29 CFR 1910.28(d)(7):     Guardrails not less than 2 x 4 inches or the equivalent and not less than 36 inches or more than 42 inches high, with a mid-rail, when required, of 1- x 4-inch lumber or equivalent, and toeboards, were not installed at all open sides on the scaffold more than 10 feet above the ground or floor.     a) Jobsite: 301 S Columbus Blvd, Philadelphia, PA. - On or about 4/27/12, an adequate mid-rail was missing from one side of the elevated platform employees were working from.  The employees were performing maintenance operations on a parking lot light post approximately 18 ft. above ground level.

29 CFR 1910.178(a)(4):     Modifications and additions which affect capacity and safe operation were performed by the customer or user without manufacturers prior written approval.     a) Jobsite: 301 S Columbus Blvd, Philadelphia, PA. - On or about 4/27/12, manufacturer prior written approval was not obtained for the company manufactured platform added to the powered industrial truck, which could affect the capacity and safe operation.  The employees were performing maintenance operations on a parking lot light post approximately 18 ft. above ground level.

29 CFR 1910.178(l)(1)(i):     The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    a) Jobsite: 301 S Columbus Blvd, Philadelphia, PA. - On or about 4/27/12, an employee operating a powered industrial truck was exposed to crush hazards associated with tip overs as a result of not wearing the supplied seat belt, and by not successfully completing a powered industrial truck training program.  The employee used the truck to elevate two workers on a platform, approximately 18ft above ground level, to conduct maintenance operations on a parking lot light post.
