OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of amputation: Two-hand operating levers belonging to the following machines were not protected against premature or accidental cycling: Inside of the 3 through 5 line: a/ NEFF Pneumatic/Hydraulic Press, s/n P9177. b/ Air-Hydraulic Press, m/n C-250, s/n 2550. c/ Air-Hydraulic Press, m/n C-250, s/n 1191/J3755. Inside of the valve cell department: a/ Denison Multi-Press, m/n 6135. b/ Denison Multi-Press, m/n T-7018. One method of abatement to correct this apparent hazard is to construct each two-hand operating lever so that it cannot be prematurely or accidentally cycled as required by ANSI B11.19-1990, paragraph 4.2.4.2.1.
