29 CFR 1926.451(f)(4): Any part of a scaffold damaged or weakened such that its strength is less than that required by paragraph (a) of this section shall be immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired. A scaffold plank on a fabricated frame scaffold had a long crack running lengthwise along the plank.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees, including a competent person, were not adequately trained to recognize defective planking.
