OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a.  On or about May 24, 2012, on the south side of the building the oxygen/acetylene hoses and torch head were not fitted with backflow protection or flashback protection, exposing employees to explosive hazards.

29 CFR 1926.350(h): Oxygen, and fuel gas pressure regulators, or their related gauges were not in proper working order while in use:      a.  On or about May 24, 2012, on the south side of the building, the oxygen gauge on the oxygen/acetylene manifold was attached to oxygen/acetylene bottles charged and in use, the oxygen gauge was damaged with a broken gauge and gauge face plate, the operator was unable to accurately read/regulate oxygen pressure, exposing employees to explosive hazards.

29 CFR 1926.350(e)(4): Manifold and header hose connections were not capped when not in use:      a.  On or about May 24, 2012, on the south side of the building, the oxygen/acetylene bottles mounted on the scissor lift had the hoses, the manifold, and the torch head left charged and unmanned, the cylinder valves were left open and not capped, exposing employees to the hazard of leaking flammable gases.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems,personal fall arrest system, or warning line system and safety monitoring system.      a.  On or about May 24, 2012, on the south side of the building, employees working on a flat roof were not protected by a fall protective system, exposing themselves to a 23-foot fall hazard.
