29 CFR 1910.303(f)(2):     Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    a) On or about 5/29/12, on the second floor of Haffey Dormitory, St. John Fisher College, Rochester, NY: There were 41 Circuit breakers in panel box LPG2 located in the hallway. Attached to the inside of the door were two different labeling lists {a new list and an old list (dated 8/13/91)} to identify the purpose of each circuit breaker. Each list referred to individual circuit breakers by number. However, each circuit breaker did not have a corresponding number next to it. There was a double column of circuit breakers in the box, and employees had to manually count the circuit breakers to ascertain its' number.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.332(b)(1):     Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    a) On or about 4/1/12 thru 6/7/12 in various areas of St. John Fisher College, Rochester, NY:  Electricians and HVAC Technicians perform installation, maintenance, & repair of electrical equipment, including voltage testing &  troubleshooting live on circuits up to 480 volts-ac where they are potentially exposed to shock and arc flash hazards which necessated the use of Personal Protective Equipment (PPE) for eyes, face, head, torso,hands, and extremities. These employees were not trained and familiar with requirements to wear appropriate PPE for the tasks they performed,  and how to select the proper PPE for the task.    b) On or about 4/1/12 thru 6/7/12 in various areas of St. John Fisher College, Rochester, NY: Electricians and HVAC Technicians perform installation, maintenance, & repair of electrical equipment, including voltage testing & troubleshooting live on circuits up to 480 volts-ac and work on de-energized circuits. These employees were not trained and familiar with the requirement to de-energize and lockout before performing electrical work and under what circumstances it is permissible to work on/near energized parts.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.333(b)(2)(iii)(A):     A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed:      a) On or about 5/29/12, on the second floor of Haffey Dormitory, St. John Fisher College, Rochester, NY: An employee was changing 120 volt-ac wall receptacles in Room 225 and other rooms. The circuit was de-energized by switching the appropriate circuit breaker to the "off" position. However, a lock and tag was not placed on the circuit breaker, and the door to the panelbox containing the circuit breaker was left wide open.    b) On or about 5/15/12, on the second floor of the Ralph Wilson Building, St. John Fisher College, Rochester, NY: An employee was changing 277 volt-ac ballasts in lighting fixtures. The circuit was de-energized by turning off the light switch. However, a lockout device and tag was not placed on the light switch nor on the circuit breaker associated with that light switch.      ABATEMENT CERTIFICATION REQUIRED

The employee was not provided with and required to use suitable rubber insulating gloves, arc- rated long sleeve shirt and pants, hardhat and safety glasses.    b) On or about 4/01/12 - 4/30/12, in Lavery Library & Kerney Hall and throughout the St. John Fisher College Campus, Rochester, NY: An employee was using a "Multi-Meter" to troubleshoot live electrical parts at 460 volts-ac on chillers such as but not limited to Motivar & Trane Chillers. The employee was not provided with and required to use suitable rubber  insulating gloves, arc-rated long sleeve shirt and pants, hardhat, safety glasses, and arc-rated flash suit hood (or equivalent).    ABATEMENT DOCUMENTATION REQUIRED
