29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: JOB SITE: 11818 Flushing Drive, Maryland Heights, MO 63043, at this work site, workers were observed on the roof of the residential structure, approximately 9 feet 9 inches eave to ground level, actively installing new roof shingles without any form of eye or face protection. The workers were exposed to a struck-by hazard.
