29 CFR 1910.1001(j)(4)(i):     The building owner, facility owner or employer identified previously installed presumed asbestos containing materials (ACM) and/or presumed asbestos containing materials (PACM) and did not affix or post labels or signs so that employees would be notified of what material contains ACM and/or PACM:    a) Mansion Building, 285 Madison Avenue, Madison, NJ 07940    The thermal systems insulation above the dropped ceiling tiles in the Hennessy Hall basement was not labeled as ACM.    Violation occurred on or about 5/31/2012.

29 CFR 1910.1001(j)(7)(iv): The employer did not provide, at no cost to employee(s) who perform housekeeping operations in an area that contains ACM or PACM, an asbestos awareness training course, which shall at a minimum contain the following elements  health effects of asbestos, locations of ACM and PACM in the building/facility, recognition of ACM and PACM damage and deterioration, requirements in this standard relating to housekeeping, and proper response to fiber release episodes, to all employee(s) who perform housekeeping work in areas where ACM and/or PACM was present, at least annually for each employee    a) Mansion Building, 285 Madison Avenue, Madison, NJ 07940    Custodial staff has potential exposure to asbestos from thermal systems insulation when cleaning the facility.  The staff did not receive awareness level training for proper cleaning methods and for recognizing the ACM in the facility.    Violation occurred on or about 5/31/2012.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:    a) Mansion Building, 285 Madison Avenue, Madison, NJ 07940    Employees with potential exposure to lead from paint dust on the eating tables in two rooms.  One lead wipe in each of the two break areas (Room #NBG and Snax Room) in the basement area of the building.  The room #NBG result was 51.5 ug/cm2 (478.5 ug/ft2).  The Snax Room result was 131.3 ug/cm2 (1219.9 ug/ft2).  Both dust sample results are above the HUD risk assessment level of 40 ug/ft2.      Violation occurred on or about 8/02/2012.
