29 CFR 1910.133(a)(1): Protective eye and face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:    (a) Tool and Cutter Grinder - The employer did not require an employee to wear safety glasses with side shields when he used the K. O. Lee Company Tool and Cutter Grinder, serial number 14726CD, to put the sharp edge on reamers.  The employee's face and eyes were exposed to grit and other flying debris from the process, on or about May 23, 2012.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) Wayne Tool Company, 274 North Franklin Street, Waynesboro Pennsylvania - Employees performed maintenance servicing activities on machinery and equipment, including but not limited to the drill flatting machines.   The employer did not develop and implement an energy control program including machine specific lockout/tag out procedures, periodic inspections and written policy thus exposing employees to various injuries on or about May 23, 2012.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.     (a) Wayne Tool Company, 274 North Franklin Street, Waynesboro Pennsylvania - The employer did not develop or document specific energy control procedures for all required equipment and machinery, thus exposing employees to to possible injuries, on or about May 23, 2012.     Abatement Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    (a) Federal Welder serial number 8632 - The employer did not ensure that the old, unused switch opening on the Federal Welder was closed.  Employees turned the welder on and off with the new switch located five inches to the right of the previous switch box and were exposed to the possibility of inserting a finger into the old switch box contacting live 115VAC current, on or about May 23, 2012.
