29 CFR 1910.303(b)(1)(vii): Electrical equipment was not free of recognized hazards that was likely to cause death or serious physical harm to employees. Safety of equipment was not determined using considerations for classification by type, size, voltage, current capacity, or specific use. At the time of the inspection, the main electrical box containing 440 volt service was operated with one of the three hot leads jumpered with copper wire instead of being protected with a current limiting fuse.
