29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  roof of Country Fair at 2617 W. 26th Street in Erie, PA.  On or about May 24, 2012, employees working on the roof of the store were not protected from falling 12 feet to the lower level.

29 CFR 1926.503(a)(2):       The employer did not assure that each employee exposed to fall hazards was trained in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (a)(2)(viii):      a) roof of Country Fair, 2617 W. 26th Street, Erie, PA.  On or about May 24, 2012, employees working approximately 12-feet above the ground on the roof of the Country Fair were exposed to a fall hazard.  The companys fall protection training program did not cover items that included but were not limited to:         1.) The 29 CFR 1926 Subpart M       2.)  Fall arrest system inspection, erection, disassembly, and maintenance procedures

29 CFR 1926.503(b)(1):     The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:    a) roof of Country Fair, 2617 W. 26th Street, Erie, PA.  On or about May 24, 2012, the employer did not have written certification to verify that the employees who were working 12 feet above the lower level had been trained in accordance with paragraph (a) of this section.
