29 CFR 1910.102(a) The in-plant transfer, handling, storage, and utilization of acetylene in cylinders was not in accordance with Compressed Gas Association Pamphlet G-1-1966, which is incorporated by reference as specified in 29 CFR 1910.6: a) At 7425 Highway 190 West, Port Allen, LA; the employer allowed acetylene cylinders to be stored in an open yard area where they are subject to direct sunlight and high temperatures without shade and/or covering. This condition was determined on or about May 23, 2012, during the investigation of a fire/explosion event that occurred at the site on May 21, 2012 where acetylene cylinders caught fire and exploded. The acetylene cylinder(s) (P/C 350-75; Barcode 011431996) directly involved in the event had been stored in the outdoor area for 20 days and was subject to hot outdoor temperatures (a high of 90 degrees Fahrenheit). Reference Standard CGA G-1--2009 Acetylene; Section 5.2: Rules for storing acetylene cylinders: When storing acetylene cylinders outdoors, they should be protected from the ground to prevent bottom corrosion. During the winter, cylinders stored in the open should be protected against accumulations of ice or snow. In summer, charged cylinders stored in the open should be screened against the direct rays of the sun in those localities where extreme temperatures prevail.

29 CFR 1910.157(e)(2) Portable fire extinguishers were not visually inspected at least monthly: a) At 7425 Highway 190 West, Port Allen, LA; the employer did not ensure that all portable fire extinguishers were visually inspected at least monthly. This condition was observed on or about May 23, 2012, in the Loading Dock Area where portable fire extinguishers (AMEREX ABC Dry Chemical, Model A411), provided for employee use, had not received a required monthly visual inspection since March 2012.

29 CFR 1910.178(c)(2)(v) Power-operated industrial trucks designated as DS, ES, GS or LPS were not used in locations where hazardous liquids in sealed containers or liquefied or compressed gases in containers were being handled and stored: a) At 7425 Highway 190 West, Port Allen, LA; the employer allowed employees to operate a diesel engine powered forklift truck in areas designated by the employer as a Class I Division 2 hazardous location. This condition was determined on or about May 23, 2012, during the investigation of a fire/explosion event that occurred at the site on May 21, 2012 where acetylene cylinders caught fire and exploded. A Harlo industrial forklift truck (Model HP8500; S/N 101449) was operated in the Maintenance/Yard area just prior to the incident, approximately 40 feet from the exploding acetylene cylinder and is used throughout the area on daily basis. The forklift truck was not approved for Class I Division 2 hazardous locations where flammable compressed gases are handled and stored.
