1926.502(h)(1)(v): The safety monitor had other responsibilities which took the monitor's attention from the monitoring function.    a) Single Source Roofing Corporation on or about May 30, 2012, at the address of 2041 Claremont Ave. Ashland, OH: The employer failed to ensure the safety monitors only responsibility was the monitoring function.  The designated worker charged with monitoring employees performing roofing work on a low slope roof, partook in activities such as removing/installing roofing material and other layout activities. These actions took his attention away from employees working near the roofs perimeter.  Employees were exposed to a potential 22 fall to the ground below.
