(a) Jobsite: 207 Hickory Street, McKees Rocks, PA: On or about May 30, 2012, an employee entered a trench to remove a barrel with near vertical walls and no cave-in protection that measured approximately 6 feet deep, and water accumulated in the bottom of the trench.

(a) Jobsite- 207 Hickory Street, McKees Rocks, PA: On or about May 30, 2012, a competent person did not inspect trench one or trench two before employees entered either trench to ensure the safety of the employees working in the two trenches with near vertical sidewalls.

(b) Jobsite- 207 Hickory Street, McKees Rocks, PA: On or about May 30, 2012, an employee entered a trench with near vertical walls to cover a sewer line pipe. The trench measured approximately 8 feet and 10 inches in depth without cave-in protection.
