29 CFR 1910.23(a)(8)(ii): While the cover was not in place, the floor hole was not constantly attended by someone or protected by a removable standard railing. On or about May 11, 2012, employees walking and working on catwalks with holes due to rust and corrosion and were exposed to falling approximately 20 feet to the ground below.

29 CFR 1910.24(c): Fixed stairways were not designed and constructed to carry a load of five times the normal live load anticipated but never of less strength than to carry safely a moving concentrated load of 1,000 pounds. On or about May 11, 2012, employees assigned to check the fluid level at the pump station were exposed to falling through stairs and platforms approximately 20 feet to the ground below.

29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons was not adequately trained to render first aid. Adequate first aid supplies were not readily available. On or about May 11, 2012, employees working in a remote location outside of the city limits were not provided an adequately supplied first aid kit. Employees were exposed to cuts and/or abrasions from rusted stairway handrails.
