29 CFR 1926.453(b)(2)(iv):  Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:    (a) 1 Crusader Way Harrisburg, PA - An employee was standing on two five gallon buckets next to each other approximately 20 feet high in a Genie S-40 aerial lift to elevate himself higher in the basket between the block wall and the HVAC to caulk the seams, thereby exposing employee(s) to injuries, on or about May 22, 2012.          No abatement certification or documentation required.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift:      (a) 1 Crusader Way Harrisburg, PA - An employee was utilizing a Genie S-40 aerial lift approximately 20 feet high preforming caulking activities and was not protected/restrained inside the basket by the use of a body belt and attached lanyard, thereby exposing employee(s) to a fall, on or about May 22, 2012.     The ABG Caulking & Waterproofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453 (b)(2)(v), which was contained in OSHA inspection number 311817050, citation number 1, item number 1 and was affirmed as a final order on November 5, 2008, with respect to a workplace located at 14900 Florencia, Perdido Key, FL 32514.    No abatement certification or documentation required.

29 CFR 1926.1053(b)(4): Ladders were not used for the purpose in which they were designed:    (a) 1 Crusader Way, Harrisburg, PA  Employee used a ten-foot Weaver fiberglass stepladder, leaned it against a block wall, and was up on the third rung to perform caulking activities, thereby exposing the employee to injuries resulting from potential inadvertent ladder movement, on or about May 22, 2012.    No abatement certification or documentation required.
