29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      The employer did not ensure training was provided for each employee working on the job site.  As a result one of the two employees did not have fall protection training and were exposed to fall hazards.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.          THIS EMPLOYER WAS PREVIOUSLY CITED FOR VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR IT'S EQUIVALENT STANDART 29 CFR 1926.501(B)(13) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312272891, AT THE LOCATION AT 4218 CABALLO CROSSING DRIVE, FLORISSANT MO, CITATION 1, ITEM 1, ISSUED ON 9/29/2008.  THIS INSPECTION BECAME FINAL ORDER ON 12/02/2008.      At the job site located at 3659 Vago Lane, Florissant MO, on 18 April 2012 the employer did not ensure employees used fall protection system to protect themselves from 10 feet falls off the roof.  As a result the employees were exposed to fall hazards.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
