29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.      a)   North side of the building, 2nd floor deck, on or about May 22, 2012, an employee was working from the outrigger of a tubular welded frame scaffold without guardrails and no other means of fall protection, exposing him up to a 14-foot fall hazard.    ABATEMENT CERTIFICATION IS NOT REQUIRED

29 CFR 1926.501(b)(1):     "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a)  North side of the building, 2nd floor, on or about May 22, 2012, employees were working from an unguarded 2nd floor stepping on/off the stairway, exposing employees to a 12 foot fall hazard.    ABATEMENT CERTIFICATION IS NOT REQUIRED

29 CFR 1926.1052(b)(1):     Except during stairway construction, foot traffic is prohibited on stairways with pan stairs where the treads and/or landings are to be filled in with concrete or other material at a later date, unless the stairs are temporarily fitted with wood or other solid material at least to the top edge of each pan. Such temporary treads and landings shall be replaced when worn below the level of the top edge of the pan.    a) North side of the building, on or about May 22, 2012, the metal stair pans and landings were not filled in with concrete, exposing employees trip and fall hazards.
