29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b). Note: If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502. Location: 171 Killingworth Turnpike, Haddam, CT The 6 in 12 pitched roof with a 15 foot 6 inch eave to ground fall distance was not equipped with a fall protection system. REPEAT STATEMENT Toolan Construction was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 109180414, Citation number 01, item number 007a, issued on June 24, 2011, with respect to a workplace located at 5 Acorn Drive, East Haddam, CT 06423 and became a final order on July 18, 2011.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Location: 171 Killiingworth Turnpike, Haddam, CT Each employee engaged in residential roofing work was not provided with a training program to instruct each employee to recognize fall hazards and the means/procedures to minimize or eliminate those fall hazards. REPEAT STATEMENT Toolan Construction was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.503(a)(3), which was contained in OSHA inspection number 109180414, Citation number 01, item number 007b, issued on June 24, 2011, with respect to a workplace located at 5 Acorn Drive, East Haddam, CT 06423 and became a final order on July 18, 2011.
