29 CFR 1910.268(h)(7): When a ladder is supported by an aerial strand, and ladder hooks or other supports are not being used, the ladder shall be extended at least 2 feet above the strand and shall be secured to it (e.g. lashed or held by a safety strap around the strand and ladder side rail). When a ladder is supported by a pole, it shall be securely lashed to the pole unless the ladder is specifically designed to prevent movement when used in this application. a) At the job site, on or about May 23, 2012, an employee was accessing an area of the guyed wires using a 24-foot extension ladder, while it was leaning against the guy wires without being secured in placed and not being 2 feet above the strand, exposing the employee to a fall hazard. ABATEMENT CERTIFICATION AND DOCUMENTATION IS NOT REQUIRED

29 CFR 1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. a) At the job site, on or about May 23, 2012, an extension cord supplying electrical power for portable hand held tools used by employees was located across driveway and subject to damage by vehicular traffic, exposing employees to electrical shock hazards.
