29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a)  Building 9 - On or about July 11, 2012, an electrical junction box was not adequately mounted on the wall  exposing live electrical components.     b)  Burner Room - On or about July 20, 2012, live electrical wires were not guarded by means of an electrical box and were setting on an accumulation of combustible paper towel dust (ST1).

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv):    a)  Building 9 Mezzanine - On or about July 11, 2012, an extension cord was used as permanent wiring for a mounted Dayton commercial air circulator (Serial # 11434159).    b)  Building 9 Mezzanine - On or about July 11, 2012, a flexible cord was used for permanently fixed lighting.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a)  Building 9 - On or around May 30 and July 11 of  2012, structural supports, pipes, fixtures, ductwork, walls, room roofing, floors and equipment were not kept free of hazardous accumulation of combustible dust.    b)  Building 8 - On or around May 30, July 11 and July 20 of 2012, structural supports, pipes, fixtures, equipment and ductwork were not kept free of hazardous accumulation of combustible dust.    c)  Building 15 - On or around May 30 and July 20 of 2012, structural supports, pipes, fixtures, equipment and ductwork were not kept free of hazardous accumulation of combustible dust.    d)  Burner/Control Room - On or around May 30 and July 20 of 2012, structural supports, pipes, equipment, and ductwork were not kept free of hazardous accumulation of combustible dust.

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:    a)  Building 9 - On or around May 30 and July 11 of 2012, employees conducted a blow down cleaning procedure for tissue paper dust, a Class II Group G Explosive Dust, suspending the dust in air.  General purpose wiring, breaker panel boxes, disconnect switches, receptacles and electric motors were not dust-tight ignition proof and approved for Class II Division 1 locations.    b)  Building 15 - On or around May 30 and July 20, 2012, employees conduct a blow down cleaning procedure for tissue paper dust, a Class II Group G Explosive Dust, suspending the dust in the air.  General purpose wiring, breaker panel boxes, disconnect switches, receptacles and electric motors were not dust-tight ignition proof and approved for Class II Division 1 locations.
