29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).    The employer does not fully plank each platform on all working levels of scaffold. This violation was observed at the construction site on May 29, 2012 where employees were exposed to a 13 foot fall hazard while working from a scaffold that was not fully planked.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    The employer does not protect employees working on scaffolds from falling more than 10 feet above a lower level. This violation was observed at the construction site on May 29, 2012 where employees without fall protection were exposed to a 13 foot fall hazard while working from a scaffold that did not have proper access.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    The employer does not provide scaffolds with a proper ladder access. This violation was observed at the construction site on May 29, 2012 where employees were exposed to a 13 foot fall hazard while working from a scaffold that did not have proper access.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    The employer does not protect employees from falling when working 6 feet or more above lower levels. This violation was observed at the construction site on May 29, 2012 where employees were exposed to a 10 foot fall hazard when performing residential construction activities without fall protection.
