29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. The employer did not certify the periodic inspections of the energy control procedures by indicating the required information: to include but not limited to the machine or equipment, the employees included in the inspection and the person who performed the inspection. Employees were exposed to various hazards including but not limited to struck-by and caught between.
