29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which was incorporated by reference as specified in 29 CFR 1910.6:    The employer failed to properly store oxygen and acetylene cylinders. Cylinders were stored between a flammable chemicals storage cabinet and a doorway through which the forklift travels.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    The employer failed to develop an energy control program that includes specific procedures, training and periodic inspection. Employees were required to operate pipelines under construction while loading and unloading product to and from the trucks. Employees were exposed to accidental releases of hot asphalt while performing these activities.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:    The employer failed to provide locking devices to secure the valves and prevent unintentional operation of the pipelines. The employer was using tags while the pipeline was under construction to indicate which lines should not be opened. Employees were not protected against an accidental release in the event the tag was removed, not noticed or missing.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903. 19(d).

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.    The employer authorized production employees to operate the powered industrial trucks failing to provide training that included a practical training, (demonstration performed by the trainer and practical exercises performed by the trainee) and an evaluation of the operators performance in the workplace.
