29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    a) Located along the rake/gable ends of the residential gambrel roof structure employees performed roofing associated operations on a 5 in 12 pitched roof without end wall roof conventional fall protection and were exposed to fall potentials in excess of 13'.

29 CFR 1926.501(c)(1): When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels:    a) Located along the perimeter exterior upper roof edge of the residential structure a ladder occupied employee performing roofing associated operations along the structures lower roof was exposed to an unseccured roll of ice guard material which was not protected from falling into/onto worker exposed lower level by a guardrail/barrier system or other equivalent method.
