Bancroft, Toledo, Ohio: On or about May 31, 2012, the employer did not ensure employees wore eye protection while operating pneumatic nail guns to secure asphalt shingles and plywood. Two employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

Reetz, Inc. worksite located at 2535 W. Bancroft, Toledo, Ohio: On or about May 31, 2012, the employer did not ensure an employee was protected from falling approximately 22 feet while standing in the bucket of a rough terrain lift being used to lower weatherproofing debris to the ground. No fall protection was being worn exposing the employee to fall hazards on the east side of the structure.

Employees were exposed to a fall distance of approximately 24 feet to the lowest level from the 5/12 pitch roof.
