29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    A. Reetz, Inc. worksite located at 2535 W. Bancroft, Toledo, Ohio: On or about May 31, 2012, the employer did not ensure employees wore eye protection while operating pneumatic nail guns to secure asphalt shingles and plywood. Two employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

Reetz, Inc. worksite located at 2535 W. Bancroft, Toledo, Ohio: On or about May 31, 2012, the employer did not ensure employees wore head protection while working below roofing activities exposing the employee to the potential of falling tools and material on the east side of the three story structure.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    A. Reetz, Inc. worksite located at 2535 W. Bancroft, Toledo, Ohio: On or about May 31, 2012, the employer did not ensure an employee was protected from falling approximately 22 feet while standing in the bucket of a rough terrain lift being used to lower weatherproofing debris to the ground. No fall protection was being worn exposing the employee to fall hazards on the east side of the structure.

Bancroft, Toledo, Ohio: On or about May 31, 2012, the employer did not ensure employees were protected from falls while performing weatherproofing activities on the three story multi-family structure. Employees were exposed to a fall distance of approximately 24 feet to the lowest level from the 5/12 pitch roof.
