29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. At the site, the employer did not make frequent inspections, nor have a competent person on the job site to ensure that employees were following the fall protection program for the site. Employees were working at heights greater than 6 feet while being unprotected from falls.
