29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) 250 Valley Brook Drive, Lancaster, Pennsylvania - An employee was working and standing on top of the primary trusses was exposed to a 20-foot fall hazard, and the employee's fall harness was not connected to a life line, on or about May 7, 2012.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      (a)  250 Valley Brook Drive, Lancaster, Pennsylvania, Wing - E  There was no training in fall protection for the employee who was working and walking on top of  the  primary trusses waiting for the secondary trusses  to be lifted and was exposed to a  twenty feet fall hazard was not tied off, on or about May 7, 2012         Abatement certification and documentation required within 10 days after abatement date.
