29 CFR 1926.602(c)(1)(vi):     Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.    A) West side of job site, on or about May 17, 2012, a GEHL forklift used to move material did not have a functioning back-up alarm, exposing employees to a struck by hazard.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.     A) West side of building, on or about May 17, 2012, employees used the scaffold support legs to access and exits the scaffold, exposing employees to a fall hazard of up to 12 feet.       Caraway ABC Construction, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.451(e) (1) which was contained in OSHA inspection number 314095316, citation number (1), item number (2), issue on 11/14/2011 and was affirmed as a final order on 12/9/2011, with respect to a workplace located at 46 Pooler Parkway, Pooler, GA

29 CFR 1926.451(g)(4)(i):     Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:    A) West side of building, on or about May 17, 2012, employees were working from a scaffold platform while the scaffold was still under construction, exposing employees to a fall hazard of approximately 18 feet.     Caraway ABC Construction, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.451(g) (4)(i) which was contained in OSHA inspection number 314096066, citation number (1), item number (1), issue on 11/14/2011, and was affirmed as a final order on 1/12/2012, with respect to a workplace located at 10 Godley Station Blvd., Pooler, GA.
