(a) Facility, New Building:  On or about May 30, 2012, there was no guardrail at the edge of the upper level.  The fall hazard to the lower level was 48 1/2 to 49 1/4 inches.

In addition, this guard was not interlocked to prevent the hydraulic ram from operating without the guard being fully closed.     (b)  Facility, New Building, Upper Level:  On or about May 30, 2012, an employee would operate the Union Environmental bailer with its guard in the full upright position.  In addition, this guard was not interlocked to prevent the hydraulic ram from operating without the guard being fully closed.
