29 CFR 1926.652(g)(2): Additional requirement for shield systems used in trench excavations. Excavations of earth material to a level not greater than 2 feet (.61 m) below the bottom of a shield shall be permitted, but only if the shield is designed to resist the forces calculated for the full depth of the trench, and there are no indications while the trench is open of a possible loss of soil from behind or below the bottom of the shield. On or about June 11, 2012 subcontractor employees were working in a trench where greater than 2 feet of earth was exposed below the bottom of the trench shield exposing employees to the hazards associated with excessively exposed unprotected trench walls. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about June 11, 2012 subcontractor employees were working in a trench in which the trench shield did was not installed per the specifications of the tabulated data. The employees were exposed to the hazards associated with improper cave-in protection. R.A.
