29 CFR 1926.350(a)(9): Compressed gas cylinders were not secured in an upright position at all times:    a) 1st Floor, Southeast Corner - A 100 pound full propane cylinder and two full 160 liter liquid oxygen cylinders were not secured in an upright position. A laborer was observed cleaning the area where the oxygen cylinders and propane cylinder were located exposing him to a potential struck-by and fire hazards.

29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:    a) 1st Floor, Southeast Corner - Two full 160 liter liquid oxygen cylinders were stored with a full 100 pound propane cylinder.  A laborer was observed cleaning the area where the oxygen cylinders and propane cylinder were located exposing him to a potential struck-by and fire hazards.

29 CFR 1926.501(c)(1):  When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels:    a) Throughout the Site on Every Floor -  Toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels were not erected. Superintendent and workers were observed on the ground level working and walking where demolition debris was stacked at edge of each the upper floors exposing them a potential struck-by hazard.

Superintendents and workers were observed entering and exiting the building where demolition was being performed and overhead protection was not provided at the entrance way exposing them to a potential struck-by hazard from falling debris.
