29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Building Roof - On or about 31 May, 2012 - Employees engaged in installing roofing shingles on the rooftop were exposed to a fall hazard when they were not protected from falling by either guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Employees were exposed to a fall height of 12 foot 6 inches.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) Building rear - On or about 31 May, 2012 - Employees were using a ladder to gain access and exit from the roof that did not have side rails or a grasping device that would extend a minimum of 3 feet above the landing. Employees were exposed to a fall hazard of 12 feet 6 inches.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    a) Building rear - On or about 31 May, 2012 - Employees were using a step ladder in a way that it was not designed to be used. Employees had leaned the ladder in its closed position up against the wall to gain access to the roof. Step ladders are designed to be used with the legs spread apart and the spreaders locked in place. Employees were exposed to a fall height of 12 feet 6 inches.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a) Building front - On or about 31 May, 2012 - Employees were using a portable sawzall to cut framing lumber on the roof. The extension cord that was powering the tool was missing the ground prong. Employees were exposed to an electrocution hazard.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.    a) Building front - On or about 31 May, 2012 - Employees were using an outlet to power electrical tools that was not ground fault circuit protected. Employees were exposed to an electrocution hazard.
