29 CFR 1926.404(b)(1)(i):     Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.    a.)  Hot Tip, Joint Forces Command, Langley-Fort Eustis, Shoppette:  On or about 05/31/12, the Miller Bobcat 250 10,500 Watt generator used to provide electrical power to portable tools and equipment was not equipped with a ground fault circuit interruptor or used with an assured grounding program.

Hot Tip, Joint Forces Command, Langley-Fort Eustis, Shoppette:  On or about 05/31/12, the employer did not ensure that an employee working at the edge of the roof 20' from the ground was protected by means of a fall protection system.
