29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b) Note: If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502. At the job site located at 4111 Domenique street, Florissant MO 63034, on 4 May 2012, the employer did not ensure the employee used fall protection while working on the roof. As a result the employee was exposed to a fall greater than 10 feet. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) At the job site located at 4111 Domenique street, Florissant MO 63034, on 4 May 2012, the employer did not ensure the employee used ladders thatwere extended at least 3 ft above the landings. As a result the employee was exposed to a fall greater than 10 feet. Abatement certification and abatement documentation is required for this violation.
