29 CFR 1910.147(c)(5)(i):  Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:    (a)  Employees were not provided with locks or tags to attach to the energy isolating devices, prior to cleaning and changing out the dicing blades and spacers on the Boyer Schultz Dicer.    (b)  Employees were not provided with locks or tags to attach to the energy isolating devices, prior to cleaning and changing out parts on the P.R. Hoffman Lappers.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(7)(i):  The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. 29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:      (a)  Employees operating the Grob Vertical Band Saw, were exposed to 1-1/2 inches of unguarded blade.    (b)  Employees operating the Clausing Machine Lathe, were exposed to the unguarded chuck.    (c)  The P.R. Hoffman Lapping Machine, Model PR-1, operator was exposed to a pinch point created by the outer drive gear, when she spun the outer drive gear with one hand while hitting an on/off switch with the other hand. Where the drive extends over other machine or working areas, protection against falling shall be provided. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws:      (a)  There was one inch of outer insulation pulled away from where the 120 volt AC flexible power cord for coolant on the Boyer Schultz Dicer, exposing the inner insulated wires to damage and thereby exposing employees to an electrical hazard.    (b)  There were six inches outer insulation pulled away from the 120 volt AC wires running into the motor on the side of the Boyer Schultz Dicer, exposing the inner insulated wires to damage and thereby exposing employees to an electrical hazard. 29 CFR 1910.178(q)(7):  Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift.
