29 CFR 1926.405(b)(2): Pull boxes, junction boxes, and fittings were not provided with covers. On or about 31 May 2012, the employer did not ensure a cover was installed on a breaker panel located within the general contractor's office, exposing workers to electrical shock or electrocution by live components at approximately 220 volts. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure all electrical panels are covered in order to protect workers from live components.

29 CFR 1926.417(b): Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized. On or about 31 May 2012, and at times prior thereto, the employer did not lockout or tagout the electrical source for a de-energized breaker panel that had the cover removed to "air out" the components which were located within the general contractor's office, exposing workers to electrical shock or electrocution by live components at approximately 220 volts. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure all de-energized electrical panels are locked out and/or tagged out during repair or maintenence to such in order to protect workers from live components.
