29 CFR 1926.405(a)(2)(ii)(B): Runs of open conductors used as temporary wiring branch circuits were located where the conductors would be subject to physical damage, or the conductors were fastened at intervals exceeding 10 feet:    Location: 25 State St Ossining NY 10562    On or about 6/1/12,  a) Branch circuits running from a panel outside to inside the building were not protected from physical damage and were not fastened at least every 10 feet. They were in direct contact with metal scaffolding and a metal extension ladder used to access the scaffolding.

The horizontal members of the scaffold frames did not meet the requirements for safe access.
