29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    (a) On or about May 2012, in the detail shop, for a worker wearing a 3M cartridge respirator while spraying Final Coat Rust Inhibitor containing Stoddard solvents. The employer did not ensure that this worker received a medical clearance prior to being provided with this negative pressure respirator.

Safety glasses don't provide adequate protection for the eyes.    (b) On or about March 2012, in the detail shop, for workers wearing safety glasses while pouring a five gallon container of Car Brite Wheel Acid Acid-Based Wheel Cleaner into another five gallon container for dilution with water.  Safety glasses don't provide adequate protection for the eyes or skin of the face.
