29 CFR 1926.451(g)(1):     Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    (a)  Henderson Biobehavioral Health Building, Fourth Floor - Employees were working on a scaffold, and the east corner of the scaffold was not equipped with guardrails, exposing employees to an approximately 40-foot fall, on or about May 11, 2012.     Harris Masonry Incorporated was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, which was contained in OSHA inspection number 314247560, Citation number 1, Item number 8c, and was affirmed as a final order on September 21, 2011, with respect to a work place located at 2500 Cassady Avenue, Huntingdon, Pensylvania 16652.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
