29 CFR 1910.22(a)(1):     All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    1. In the kitchen of the restaurant, the floor on which employees work was filthy.    2. In the kitchen and food preparation area, the four (4) waffle makers were dirty and in need of replacement.    3. In the kitchen and in the dishwashing area, the garbage disposal was inoperable. Employees report that the garbage disposal has not worked in five (5) years.    4. In the kitchen and in the dishwashing area, the floor under the sink was dirty and in need of cleaning.     5. In the kitchen and the food preparation area, the stoves used to cook food were in need of cleaning.    6. In the kitchen, near the Berkel food slicer, there was food particles located inside of the junction box used to energize the cooling table.    7. Inside of the men's restroom, there was a large hole in the ceiling. Employees report that water is falling into the restroom after it rains.

29 CFR 1910.212(b):     Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving:  (a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY)    1. In the kitchen, employees were using a Berkel food slicer that was not anchored to the metal table it was situated on. The slicer has fallen from the table and struck employees.

29 CFR 1910.263(e)(1):     Horizontal dough mixers with external power applications did not have all belts, chains, gears, pulleys, spockets, clutches, and other moving parts completely enclosed.    1. In the kitchen near the rear wall, employees were operating a Hobart mixer with no lid guard in place to prevent employees from making physical contact with moving parts such as the large agitator/beater.

29 CFR 1910.303(g)(1):     Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    1. In the northeast corner of the kitchen, there was a 110/220 volt electrical panel box that was blocked. The panel box was blocked by a box, bucket, mop and a hand truck.

In the northwest corner of the kitchen, there were seven (7) openings in the middle panel box with ten (10) breakers missing from the 110/220 volt panel box.    2. In the kitchen, near the Berkel food slicer, there was a cover missing from the junction box that energizes the cooling table.
