29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:      a)  On or about June 4, 2012, an employee was exposed to fall hazards in excess of 30 feet while performing window glazing on the exterior of the building.  The employee climbed over the aerial lift basket railing to reach the area where he was working.       Abatement Note:  Abatement certification is required for this item (see enclosed Sample Abatement-Certification Letter).

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) On or about June 4, 2012, an employee exposed to fall hazards in excess of 30 feet, was not provided training on proper use of fall protection and anchor points.
