At a jobsite located adjacent to 2421 SW Adams, Peoria, IL, employees installing decking material were exposed to fall hazards and the employer failed to ensure that frequent and regular inspections of the jobsite were made to by a competent person to identify and eliminate the hazards.

At a jobsite located adjacent to 2421 SW Adams, Peoria, IL, Employees removing and installing decking material were tied off to the same anchorage point which was not capable of supporting 5000 pounds per employee attached.
