29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    A.	B & B Box Company, located at 26490 Southpoint Rd., Perrysburg, OH 43551: On or about June 7, 2012, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy on EMBA Flexo Folder & Gluer (Machine # 653056 & 653200) located in the operations area.  Employees did not control the hazardous energy while performing activities such as, but not limited to, cleaning, oiling/greasing, and changing dyes on the machine without locking and tagging it out.    B.	B & B Box Company, located at 26490 Southpoint Rd., Perrysburg, OH 43551: On or about June 7, 2012, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy on F & E (Finch & Emerich) Letter Press located in the operations area.  Employees did not control the hazardous energy while performing activities such as, but not limited to, changing knifes, setting up, and making adjustments to the machine without locking and tagging it out.    C.	B & B Box Company, located at 26490 Southpoint Rd., Perrysburg, OH 43551: On or about June 7, 2012, the employer failed to control hazardous energy on Up & Down machine (built by S&S Corrugated Paper Machinery Co., Inc.) located in the operations area.  Employees did not control the hazardous energy while performing activities such as, but not limited to, changing blades and making adjustments to the machine without locking and tagging it out.    D.	B & B Box Company, located at 26490 Southpoint Rd., Perrysburg, OH 43551: On or about June 7, 2012, the employer failed to control hazardous energy on the Band Saw (built by Global Container Equipment Ltd., Model: Primo 36, Serial # AB1786) located in the operations area.  Employees did not control the hazardous energy while performing activities such as, but not limited to, changing blades and making adjustments to the machine without locking and tagging it out.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.178(l)(1)(i):     The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    B & B Box Company, located at 26490 Southpoint Rd., Perrysburg, OH 43551: On or about June 7, 2012, the employer did not ensure that employees, operating forklifts TCM (Model FCG25T7T, S/N A15G02682) and TCM (Model FCG25T7T, S/N A15G03116), are competent to complete the task, exposing employees to struck-by, caught in between, and tip over hazards. The employer did not ensure that each forklift operator received formal instructions and practical training on how to operate a powered industrial vehicle, as well as operators performance was not evaluated.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees were exposed to struck-by and caught-in-between hazards. B & B Box Company, located at 26490 Southpoint Rd., Perrysburg, OH 43551: On or about June 7, 2012, the employer failed to ensure that unused section of the blade is guarded while employees operated the Band Saw (built by Global Container Equipment Ltd., Model: Primo 36, Serial # AB1786), located in the operations area, to cut corners of boxes. During the band saw operation, the employees hands were located in a close proximity to the unguarded blade, exposing them to amputation hazards.    B.	B & B Box Company, located at 26490 Southpoint Rd., Perrysburg, OH 43551: On or about June 7, 2012, the employer did not ensure that one or more methods of machine guarding were provided to protect operators of F & E (Finch & Emerich) Letter Press, located in the operations area, from ingoing nip points created by moving parts such as ink rollers.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
