Employees did not control the hazardous energy while performing activities such as, but not limited to, cleaning, oiling/greasing, and changing dyes on the machine without locking and tagging it out.    B.	B & B Box Company, located at 26490 Southpoint Rd., Perrysburg, OH 43551: On or about June 7, 2012, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy on F & E (Finch & Emerich) Letter Press located in the operations area.
