29 CFR 1910.1030(c)(1)(iv):     The Exposure Control Plan was not reviewed and updated at least annually:    a)  The Exposure control plan for the company was reviewed and updated with changes on April 2, 2012 approxmately 13 months after the previous review on March 7, 2011.    SSC Greenfield Operating Company, LLC under Sava Senior Care was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1030(C)(1)(iv) which was contained in OSHA inspection number 314063157, citation number 2, item number 2 and was affirmed as a final order on January 11, 2012, with respect to a workplace lacated at 438 12th Street, Pawnee City, Nebraska 68420.

29 CFR 1910.147(c)(4)(ii):     Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):    a)  The energy control procedures for equipment being serviced and maintained such as air handlers, the ice machine or air compressor did not include all specific procedural steps for verifying the effectiveness of the lockout performed, removal of lockout devices and restarting of the equipment with multiple energy sources.
