29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    At this excavation site, employees preparing to install a six inch sewer pipe were not wearing a hard hat to protect them from spoil pile material striking employees on the head while working inside the excavation.

29 CFR 1926.651(c)(2):     A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    At this excavation site, employees installing a sewer line were not provided with a safe means of egress from a trench 5.3 ft. to 5.7 ft. deep by 5 ft. wide by 5.7 ft long. Neither a ladder nor an adequate slope was available for egress.

29 CFR 1926.651(j)(2):     Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary.    At this excavation site, employees installing a sewer line entered an excavation 5.3 ft. to 5.7 ft. deep by 5 feet wide by 5.7 ft. long. The spoil pile was placed at one edge of the trench and not 2 feet away from the trench. Employees were exposed to being struck by spoil pile material. The spoil pile was required to be either removed or placed at least 2 ft. away from the edge of the trench.

29 CFR 1926.652(a)(1):     Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the walls of the trench were not sloped in accordance with appendix B of 1926.652.    At this excavation site, employees entered an excavation which was approximately 5.3 ft. to 5.7 ft. deep by 5 ft. wide by 5.7 ft. long. to cut an existing line and prepare to install new six inch sewer line. The vertical walls were not shored or protected to prevent a cave-in.

The employer had not designated and trained a person to be a competent person for trenching and excavation operations.
