29 CFR 1910.176(f):     Derail and/or bumper blocks were not provided on spur railroad tracks where a rolling car could contact other cars being worked or enter a building, work or traffic area:     At this location, employees involved in a scrap metal operation were weighing rail gondola cars on the spur rail tracks inside of the employer's property.  The employer did not ensure derail and/or bumper blocks were available and used to prevent coupled rail gondola cars from rolling.

29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed:    Employees were exposed to electrical shock due to unused openings in the electrical breaker panels being left open:    a.  General Electric breaker panel on the inside wall at the mechanic shop.    b.  110 volt electrical outlet junction box at the end of a flexible electrical cord on work table in north end of mechanic shop.     c.  The Tram-A-Lot baler, was in the process of waiting for a new electrical switch to be installed and the opening where the switch had been located was left open with live open with live electrical conductors present.  The Square D Main Disconnect, Number 10 breaker and Number 03 breaker which supplied power to the Tram-A-Lot baler, was energized and was not locked out.

29 CFR 1910.303(b)(6):     Conductors or equipment located in damp or wet locations; where exposed to gases, fumes, vapors, liquids, or other agents that had a deteriorating effect on the conductors or equipment; or where exposed to excessive temperatures:    a.  The employer had installed a rectangular electrical wiring box at the bottom of the electrical meter with the meter loop on the east side of the mechanical shop which was exposed to corrosive atmospheric conditions such as but not limited to high humidity, salty atmosphere and torential rains.  This rectangular electrical wiring box served as an electrical wiring trough with live electrical conductors of 110 volts and 220 volts and was rusted through at the bottom.       b.  A General Electric disconnect box, was located adjacent to he electrical meter.  The small lip at the bottom of the cover was completely eaten away by corrosion which did not permit a lock to be placed in order to prevent entry by unauthorized personnel into the live 200 Amp, 240 volt electrical box.

29 CFR 1910.305(g)(2)(ii):     Flexible cords were not used only in continuous lengths without splice or tap:    Employees were exposed to electrical shock from flexible electrical cords where the exterior insulation was missing and there was no rubberized insulated tape present to cover the actual copper conductors:    a.  110 volt, AC flexible electrical cord located in the workroom at the north end of the mechanic shop.    b.  230 volt, DC flexible electrical cord located on the top of the magnet of the Sennebogan, Model Number 825.

Flexible electrical cord which entered a Hendley & Whittemore Model Number G-2 WEG electrical box without a functional strain relief device.      b.  Flexible electrical metal conduit with 110 volt conductors to a two outlet receptacle adjacent to the entry on the east side of the mechanic shop.
